Check box form fields can
be used for any activity or form that provides
a multiple choice answer format and are particularly
useful where the answer requires more than
one option to be selected from a list.
This example shows
boxes beside each statement. The
learner must click onto the box and it will
become ‘checked’. This particular
example shows three boxes that have been checked.
The following instructions
will help you create the simple check box activity
shown above.
- Open Word and start with
a new blank document.
- Open the Forms toolbar.
Go to View > Toolbars > Forms.
- Insert the title ‘Checklist
for ICT Skills’ and below this text
type the following:
Word Processing Skills
Tick if you can…
Use the basic features of a word processing
programme.
Open, edit and save documents.
Use formatting techniques such as headings,
emphasis, fonts and font sizes.
Insert a picture (Clip Art or by copying
and pasting images).
- Place the cursor
in front of ‘Use
the basic features…’ and click
onto the Check Box Form Field icon
(
)
on the Forms toolbar. This
will place a grey box at the mouse insertion
point. Repeat at the start of the next
three lines.
- To activate, and confirm
the check box form fields select the Protect
Form (
)
icon from the Forms toolbar.
Click onto a grey box and see if the check
symbol (X) is fully visible. Click back onto
the Protect Form icon.
- To fully protect the document
so that it avoids any accidental amendment
by a user go to Tools > Protect
Document. Type in a password to
allow amendment.
- Save the document as ICT
skills checklist to your E-Guides
USB memory stick.
